Amplify strives to provide a seamless purchasing experience. 

To submit your order, you will need an authorized Purchase Order (PO) or Letter of Intent (LOI) which includes your price quote number, located on the quote provided by your account executive. If you are a new customer, please also submit a copy of your tax exemption certificate within this form.

Check to ensure your PO includes:

1. Signature and date
2. Matching total from price quote (shipping and handling is included, if applicable)

Check to ensure your LOI includes:

1. School letterhead
2. An authorized signature
3. Matching total from price quote 
4. Price quote number
